Zusammenfassung
Die orthotope Lebertransplantation ist für Patienten mit terminaler Leberinsuffizienz bzw. malignen Tumoren zu einer Behandlungsmethode mit überschaubarem Erfolg und vertretbarem Risiko geworden [2, 12]. Die Durchführung der Narkose ist nur eine der Aufgaben des Anästhesisten; vielmehr muß er sich über ein kontinuierliches hämodynamische Monitoring und eine engmaschige Kontrolle biochemischer Parameter darauf einstellen, die Auswirkungen eines enormen Operationstraumas, das einen sehr oft schwerstkranken Patienten trifft, über eine entsprechende Volumenstherapie, Blutersatz, Gerinnungsoptimierung und verschiedene pharmakologische Interventionen zu minimieren. In der Folge sollen basierend auf eigenen Erfahrungen bei 86 Lebertransplantationen die Probleme der präoperativen Gerinnungsoptimierung diskutiert sowie das eigene Anästhesie- und intensivmedizinische Überwachungsregime beschrieben werden.
