Zusammenfassung

Funktionelle kardiovaskuläre Beschwerden stellen in der Bevölkerung eine häufig vorkommende Erkrankung dar. Nach verschiedenen Schätzungen dürften bei 30–40% der Patienten mit Herz-Kreislauf-Beschwerden die Beschwerden funktionell bedingt sein. Die Patienten sind durch die Beschwerden z. T. erheblich beeinträchtigt, der Umfang und die Intensität der Beschwerden sind meist größer als bei Patienten mit organisch bedingten Herz-Kreislauf-Beschwerden. Die Entstehung wurde bisher auf verschiedene Weise erklärt, wobei körperlichen Prozessen, wie einer sympathikoton oder vagoton labilen Regulationslage, ein unterschiedliches Gewicht zugemessen wurde. Dabei wurden funktionelle Beschwerden z. T. unterschiedlich gegenüber anderen Erkrankungen, wie etwa der Hypertonie, abgegrenzt. Übereinstimmung herrscht jedoch in der Annahme, daß einer körperbezogenen Angst sowie einer depressiven Stimmungslage eine zentrale Bedeutung für die Entstehung und Aufrechterhaltung der Beschwerden zukommt.

Abstract

For nearly 40% of all patients with severe complaints, no somatic causes of the symptoms can be found. The pathogenesis of this functional disorder has been described in different ways; some authors stress somatic factors, some psychological, some both, meaning lability of regulation. These concepts are discussed, together with the role of anxiety and depression which are commonly regarded as basic factors in the illness.
