Abstract
Operable stages of testicular germ cell tumors (stages I and IIA/B) can be cured with current standard therapies. The aim of newer investigations is to lower therapyinduced morbidity whilst maintaining the same cure rates. The main problems concern inadequate sensitivity and specificity of radiological staging procedures. In clinical stage I, survival rates with surveillance and with modified lymphadenectomy (LA) are comparable. However, a randomized study is necessary to evaluate the benefits and disadvantages of both strategies. The basic measure for such a study would be the quality of life. Prognostic factors should be found which are better able to define stage I and which therefore allow more specific procedures. In stage II A/B, LA without adjuvant chemotherapy results in a 50% relapse rate. However, cure rates are comparable to those achieved by standard therapy, i.e., radical LA plus two cytostatic courses. With primary chemotherapy, cure rates of 90% are obtained, and only in 25% is salvage LA necessary, sparing surgical complications and ejaculation loss for most of the patients. However, at least three courses of cytostatic treatment are necessary, and those patients who have a falsepositive diagnosis receive unnecessary chemotherapy. In result, a randomized study must establish the optimal treatment regimen for stage II A/B as well.
Zusammenfassung
Die operablen Stadien des nichtseminomatösen Hodentumors (Stadium I und II A/B) können mit den heute gültigen Standardkonzepten geheilt werden. Ziel der klinischen Forschung ist es, die Therapiemorbidität bei gleichbleibender Heilungschance zu senken. Problematisch ist dabei die mangelnde Sensitivität und Spezifität der Radiodiagnostik bei der Stadienfestlegung. Im klinischen Stadium I erreicht die Surveillance-Stratégie mit der modifizierten Lymphadenektomie (LA) vergleichbare Überlebensraten. Allerdings müßte eine randomisierte Studie die Vor- und Nachteile dieser beiden Strategien abwägen. Bemessungsgrundlage einer solchen Studie wäre die Lebensqualität. Es sollten Risikofaktoren bestimmt werden, die das Stadium I besser definieren und somit ein differenzierteres Vorgehen erlauben. Im Stadium II A/B führt der Verzicht auf eine adjuvante Chemotherapie nach LA zu einer 50%igen Rezidivrate. Die Überlebensraten sind jedoch mit der Standardtherapieradikale LA + 2 Kurse Chemotherapie — vergleichbar. Die primäre Chemotherapie ergibt Heilungsraten von 90% und erspart 75% der Patienten die LA; allerdings sind hierfür mindestens 3 Kurse Chemotherapie erforderlich. Die dem pathohistologischen Stadium I angehörenden Patienten erhalten überflüssigerweise eine zytostatische Behandlung. Eine randomisierte Studie wird auch für dieses Stadium das optimale Therapiekonzept ermitteln müssen.
