Abstract
Treatment of renal transplant patients with neoplastic disease requiring chemotherapy has been poorly reported. The case of a 31-year-old renal transplant patient with neoplastic disease is presented. He developed a classical seminoma (stage I, T3 N1 MO, Beta-HCG negative and alpha-1-fetoprotein negative, immunohistological cytoceratine positive after 7 years of hemodialysis therapy and 1 year after renal transplantation and immunosuppressive therapy with azathioprin and methylprednisolone. He was treated by semicastration and conventional radiotherapy with 40 Gy. Fulminant metastases of the former seminoma appeared 3 years after renal transplantation. Aggressive chemotherapy with platinum, bleomycin, and etoposide was started and led to complete remission. Function of the transplanted kidney remained stable. Subsequent severe pancytopenia was due to lack of folic acid, diagnosed by bone marrow biopsy and serum folic acid levels, caused by the earlier immunosuppressive therapy with azathioprin and lack of substitution during intravenous nutrition. After sufficient substitution with folic acid the pancytopenia vanished. CMV infection of the lung led to death.
Zusammenfassung
Über die cytostatische Therapie von NTPL-Empfängern mit Neoplasien gibt es nur spärliche Berichte. Es wird über einen 31jährigen nierentransplantierten Patienten berichtet, der nach 7jähriger Hämodialysetherapie transplantiert wurde, ein Jahr nach NTPL unter Dauerimmunsuppression mit Imurek und Urbason ein Seminom entwickelte (Tumormarker negativ, immunhistologisch Cytokeratin positiv). Die Therapie des Seminoms bestand in einer linksseitigen Semikastration (T3 N1 MO) und in einer typischen Radiatio mit 40 Gy GHD. 3 Jahre nach NTPL kam es zu einer generalisierten Metastasierung. Die aus den Metastasen gewonnene Histologie bestätigte die Diagnose des vor 3 Jahren therapierten Primärtumors vom Cytokeratinpositiven Typ, jetzt klinisches Stadium IVB. Eine aggressive Therapie mit Bleomycin, Etoposid und Cisplatin in ultrahoher Dosis führte zu einer kompletten Remission der Tumorerkrankung bei erhaltener Transplantatfunktion. Die langanhaltende Panzytopenie hatte ihre Ursache nicht allein in der aggressiven Chemotherapie, sondern in einer ausgeprägten megaloblastären Ausreifungsstörung des Knochenmarks, bedingt durch einen Folsäuremangel, ausgelöst durch die folsäurehemmende vorangegangene Behandlung mit Imurek und mangelnde Substitution unter parenteraler Ernährung. Durch adäquate Substitution mit Leucovorin normaliserte sich das Blutbild. Der Krankheitsverlauf wurde durch eine letale CMV-Infektion terminiert.
