Abstract
From July 1984 to March 1987, 31 patients with nonseminomatous malignant testicular tumors in the disseminated stage were studied. For this investigation, chemotherapy with a sequentially alternating, non-cross-resistant combination of cytostatic agent was employed (velbe/bleomycin-adriamycin/cisplatin, ifosfamid/VP 16–213; leucocyte nadir adapted; the intervals between therapy were reduced from the usual 21–28 days to 12 +/- 2 days). The rate of response was 90.3%. Complete remissions were observed in 89.3% of cases, and the recurrence-free time is now 25 months.
The efficacy of this procedure can be compared with that of other combinations of cytostatics used in treating the above-mentioned patients. Complete remission, however, is obtained in the short-interval system after an average of 2.5 cycles. Moreover, this medication has relatively low toxicity. Preliminary results on the lowest white blood cell count with sequentially alternating short-interval chemotherapy are encouraging. This may present a new form of treatment for metastazing testicular tumors.
Zusammenfassung
Von Juli 1984 bis März 1987 wurde bei 31 Patienten mit nichtseminomatösen malignen Hodentumoren im disseminierten Stadium eine Chemotherapiestudie mit einer sequentiell alternierenden, nichtkreuzresistenten Zytostatika-Kombination durchgeführt (Velbe/Bleomycin-Adriamycin/Cisplatin-Ifosfamid/VP 16–213; Leukozyten-Nadir-adaptiert: Verkürzung des therapiefreien Intervalls von herkömmlich 21–28 Tage auf 12 +/- 2 Tage). Dabei konnte eine Gesamtansprechrate von 90,3% erreicht werden. Komplette Remissionen wurden in 89,3% der Fälle beobachtet. Die rezidivfreie Überlebenszeit beträgt derzeit 25 Monate. Die Effektivität dieses Therapieschemas ist mit anderen bei diesen Kranken eingesetzten Zytostatika-Kombinationen vergleichbar, jedoch kommt es bereits im Mittel nach 2,5 Zyklen zur kompletten Remission. Darüber hinaus ist die Toxizität relativ gering. Aufgrund der bisherigen Ergebnisse halten wir die sequentiell alternierende, am Leukozytentiefstwert orientierte, intervallverkürzte Chemotherapie für eine Bereicherung in der Behandlung des metastasierten Hodentumors.
