Abstract
The prognosis and the aggressiveness of treatment required for testicular germ cell tumors are both related to stage at presentation. Delay in diagnosis may affect the stage and prognosis. We therefore undertook a partly prospective study to measure the delay(s) in diagnosing testicular cancer among 125 patients (age 27.6 years, range 16–57) with testicular cancer (20 seminomas, 105 nonseminomas) currently attending our in- and out-patient clinic. All patients have been interviewed personally by one of us. The mean patient delay was 4.31 months (median 2.5), the mean physician delay was 30 days (median 7), and the mean total delay was 4.95 months (median 2.5). The delay caused by the hospital (first attendance to orchidectomy) was negligible. We found further a correlation between length of delay and tumor stage at presentation. The total mean delay in stage I patients was 2.75 months, in stage II 4.16 months, and in stage III 6.75 months. A review of the literature revealed that a long diagnostic delay is a significant prognostic factor only for patients with nonseminomas.
Zusammenfassung
Prognose und Therapieintensität der Hodentumoren sind vom Stadium bei der Diagnose abhängig. In dieser Studie wurden die Verzögerung und ihr Einfluß auf das Tumorstadium bei der Diagnose des Hodentumors bei 125 Patienten (mittleres Alter 28 J. (16–57 J) mit Hodentumoren (20 Seminome, 105 Nicht-Seminome) untersucht. Der mittlere Wert der durch die Patienten verursachten diagnostischen Verzögerung war 4,31 Monate (median 2,5). Die mittlere Verzögerung durch die Hausärzte betrug 30 Tage (median 7), die Verzögerung durch das Krankenhaus war gering. Die mittlere Gesamtverzögerung (durch Patienten, Hausärzte, und Krankenhausärzte) war 4,95 Monate (median 2,5). Wir fanden ferner eine deutliche Korrelation zwischen Länge der Verzögerung und Tumorstadium bei der Diagnosestellung: So war die mittlere Gesamtverzögerung, ungeachtet des Tumortyps, bei Patienten mit Stadium I 2,75, mit Stadium II 4,16, und mit Stadium III 6,75 Monate. Anhand der eigenen Ergebnisse und des Literaturvergleichs scheint die diagnostische Verzögerung ein wichtiger prognostischer Faktor nur bei Nicht-Seminomen zu sein.
