Zusammenfassung
Trotz Stadien- und histologiegerechter Operationsverfahren liegt beim Magencarcinom mit magennahen Lymphknotenmetastasen (pT 1–2 N1 M0) die Fünfjahres-Überlebensrate nur bei etwa 40 Prozent, neben peritonealen Metastasen führen vor allem Fernmetastasen zum Tod. Man muß daher annehmen, daß zum Zeitpunkt der Erstbehandlung häufig bereits eine hämatogene Tumorzellaussaat stattgefunden hat. Diese Fernmetastasen können aber mit den bisherigen Untersuchungsverfahren nicht erfaßt werden, da Sonographie, Röntgenaufnahmen, Computertomographie und Skelettszintigraphie Metastasen erst ab einer Größe von 1 Kubikzentimeter (entspricht 109 Zellen) erkennen lassen.
