Zusammenfassung
Bei Patienten, deren Kardia- oder Ösophaguskarzinom operativ behandelt wird, handelt es sich häufig um Patienten mit zahlreichen Risikofaktoren [19]. Hierzu zählen insbesondere eine eingeschränkte Lungenfunktion, ein reduzierter Ernährungs- und Allgemeinzustand und vermehrter Alkoholkonsum. Allgemeine Begleiterkrankungen wie koronare Herzerkrankung, Diabetes mellitus, Hypertonus und andere kommen hinzu. Für den perioperativen Verlauf ist die Lungenfunktion von entscheidender Bedeutung. Bei abdomino-thorakalen Eingriffen wird die Atemmechanik des Patienten erheblich beeinträchtigt. Zur Beurteilung des Risikos postoperativer pulmonaler Komplikationen werden präoperativ neben der arteriellen und kapillären Blutgasanalyse dynamische Lungenvolumina herangezogen [20]. Die forzierte Vitalkapazität (FVC) hängt besonders von der Kraft der Inspirationsmuskulatur und von der elastischen Retraktionskraft der Lunge ab. Eine ausgeprägte restriktive Ventilationsstörung mit einer FVC unter 50% läßt bei 30% der Patienten pulmonale Komplikationen erwarten [11]. Die exspiratorische Einsekundenkapazität (FEV, Tiffeneau-Test) ist ein direktes Maß des Schweregrades der wesentlich häufigeren Obstruktion. Präoperative Atemtherapie mit „incentive spirometry“ oder „coached respiration“ dienen nicht so sehr der Verbesserung der Lungenfunktion als dem Training der Atemmuskulatur. Durch Applikation von Beta-Sympathikomimetika und Sekretolytika sollte präoperativ die obstruktive Komponente einer chronischen Lungenerkrankung verbessert werden [8].
