Zusammenfassung
Das Corticotropin-Releasing-Hormone (CRH), ein aus dem Hypothalamus isoliertes Peptid mit 41 Aminosäureresten, ist der primäre Neuroregulator der Sekretion der von Proopimelanokortin (POMC) abgeleiteten Peptide, Kortikotropin (ACTH) und β-Endorphin, aus dem Hypophysenvorderlappen (Vale et al. 1981). Funktionelle Tests mit CRH erlauben die Erforschung der zentralen Kontrolle sowie die kybernetische Regulation des Hypothalamus-Hypophysen-Nebennierenrinden-(HHN)-Systems bei endokrinologischen Erkrankungen wie z. B. dem Cushing-Syn-drom (Gold et al. 1984). Die Messung einer erhöhten basalen Kortisolsekretion sowie der Nachweis einer unvollständigen Suppression des Kortisols nach Gabe von Dexamethason ließen bei einem Teil der Patienten mit endogener Depression auf eine Hyperaktivität der HHN-Achse schließen (Carroll 1982). Die Pathophysiologie der Nonsuppression von Kortisol nach Dexamethason sowie die Lokalisation des Defekts des Regelkreises im HHN-System wird jedoch noch kontrovers diskutiert (Pfohl et al. 1985). Zur Prüfung der Integrität des HHN-Systems wurde in dieser Studie der CRH-Stimulationstest bei 12 Patienten mit endogener Depression und bei einer für das Alter und Geschlecht parallelisierten Kontrollgruppe durchgeführt.
