Zusammenfassung
Schwangerschaften bei Frauen mit kongenitalem Antithrombin III (AT III)-Mangel sind mit einem sehr hohen Risiko einer thromboembolischen Komplikation verbunden. Die Inzidenz einer thromboembolischen Erkrankung ohne prophylaktische Behandlung liegt bei etwa 70% der Patientinnen [4, 8]. Prophylaktische Maßnahmen sind zum möglichst frühen Zeitpunkt der Gravidität angezeigt. Orale Antikoagulantien sind zumindest in der Frühphase der Schwangerschaft kontraindiziert. Die „low dose“ Heparinprophylaxe erscheint nicht ausreichend, ist unkontrollierbar und führt möglicherweise zum AT III-Verbrauch. Die subkutane Heparinbehandlung in hoher Dosierung konnte eine Thrombose nicht bei allen schwangeren Patientinnen mit kongenitalem AT III-Mangel verhindern [5].
