Zusammenfassung
Es wurde eine weltweite Umfrage über Methoden und Ergebnisse der Photopatchtestung durchgeführt, bei der 22 Zentren anhand eines detaillierten Fragebogens Auskunft erteilten. Unter den routinemäßig getesteten Substanzen wurden halogenierte Salicylanilide, Chlorpromazin, p-Aminobenzoesäure, 6-Me- thylcumarin und neuerdings Ambrette Moschus am häufigsten als Photoallergene gefunden.
Die Methodik des Photopatchtests variiert beträchtlich, jedoch werden einheitlich als Kontrollen ein unbelichteter Patchtest und eine Probebestrahlung auf unbehandelter Haut durchgeführt. Übereinstimmend verwenden alle Untersucher UV-A zur Belichtung der Testfelder. Die Strahlendosis reicht von 1,25 bis 12,0 J/cm2, liegt jedoch meist zwischen 2,0 und 10,0 J/cm2. Einige der Autoren verwenden zusätzlich Suberythemdosen von UV-B. Die Testsubstanzen werden okklusiv für 24 bis 48 Std appliziert. Ablesungen erfolgen 24 oder 48 Std nach der Belichtung, an einigen Instituten auch sofort und nach 72 Std.
Die Ergebnisse werden kurz diskutiert, und es werden Empfehlungen zur praktischen Durchführung des Photopatchtests gegeben.
Summary
In a world-wide survey 22 investigators contributed by answering a detailed questionnaire concerning methodology and test results of photopatch testing.
Among the routinely tested agents and the mostly identified photoallergens halogenated salicylanilides, chlorpromazine, p-aminobenzoic acid, 6-methylcoumarin and, recently, musk ambrette ranked highest. Photopatch test procedures are greatly diversified, however, there is agreement in employing non irradiated patches and irradiated normal skin as controls. In all centers questioned, test sites were irradiated with UV-A. Doses ranged from 1.25 to 15.0 J/cm2 and were mostly between 2.0 and 10.0 J/cm2. Some investigators administered additionally UV-B in suberythematous doses. Test materials were applied occlusively for 24 or 48 hours. Reactions were graded 24 or 48 hours following irradiations, in some instances also immediately after UV-exposure and 72 hours later.
The results are briefly discussed and recommendations as to a useful method of photopatch testing are given.
