Zusammenfassung
Die menschlichen Aktinomykosen sind zur Progredienz neigende, nicht übertragbare, bakterielle Infektionskrankheiten, die das männliche Geschlecht bevorzugen. Ihre wichtigsten Erreger sind Actinomyces israelii, Actinomyces naeslundii, Actinomyces viscosus und Arachnia propionica. Actinomyces bovis ist entgegen früherer Ansicht nicht humanpathogen. Eine Verringerung der Aktinomykose-Incidenz ist seit Beginn der Antibiotica-Ära nicht feststellbar. Lediglich schwere oder tödlich verlaufende Formen sind heute selten geworden. Nach gegenwärtigem Wissensstand ist anzunehmen, daß sich die Virulenz der Acti- nomyceten nicht ändert, wenn sie von der epiphytären in eine parasitäre Lebensweise übergehen. Es sind vielmehr akzidentelle bahnende Einflüsse, die die potentiell pathogenen Epiphyten zur Invasion des Gewebes befähigen. Die Diagnosestellung erfolgt mikroskopisch oder kulturell unter Verwendung geeigneter Nährmedien und anaerober Züchtungsbedingungen. Die Therapie besteht in möglichst radikaler, chirurgischer Sanierung und gezielter, hochdosierter Antibiotica-Medikation. Als Mittel der Wahl gelten Ampicillin und andere Aminopenicilline. Zur Miterfassung der Begleitflora kommt die zusätzliche Gabe von Metronidazol oder Clindamycin in Betracht. Die Prognose der Aktinomykosen ist heute als uneingeschränkt günstig zu bewerten. Eine möglichst frühzeitige Diagnosestellung ist jedoch anzustreben, um funktioneil oder kosmetisch störende Defektheilungen zu verhüten.
Summary
Human actinomycoses are non-transmissible, progressive, bacterial diseases favouring the male sex. Their most important causative agents are A ctinomyces israelii, A ctinomyces naeslundii, A ctinomyces vis- cosus and Arachnia propionica. In contrast to previous views, Actinomyces bovis is not a human pathogen. A decrease in the incidence of actinomycotic infections has not been observed since the beginning of the era of antibiotics. Only severe or lethal forms have become rare nowadays. According to the present state of knowledge, it can be assumed that the virulence of actinomycetes does not change when they turn from epiphytes into parasites. However, accidental facilitating influences are necessary to enable the potentially pathogenic epiphytes to invade the tissue. Diagnosis is established microscopically or by culture on suitable media incubated anaerobically. Treatment consists of surgical measures which should be as radical as possible, and the application of suitable antibacterial drugs in high doses. Ampicillin and other aminopenicillins are considered the drugs of choice. In order to cover the concomitant flora, metronida- zole or clindamycin may be used in addition. Today, the prognosis of actinomycoses is generally favourable. Nevertheless, the diagnosis should be established as early as possible in order to prevent functionally or cosmetically troublesome, partial recoveries.
