Zusammenfassung

Beim Lichen ruber planus (LP), einer klinisch und histologisch eindeutig definierten, ätiopathogenetisch aber immer noch ungeklärten entzündlichen Dermatose, gibt es viele klinische und laborchemische Hinweise auf ursächliche oder assoziierte immunologische Phänomene. Die Prädominanz, das gemeinsame Vorkommen und der enge Kontakt von T-Lymphocyten und Makrophagen/Langerhans-Zellen im der- mo-epidermalen entzündlichen Infiltrat, und die hydropische Degeneration der basalen Keratinocy- ten — mit Zerstörung der dermo-epidermalen Junktionszone — lassen pathogenetisch. an eine (auto)im- munologische Genese der Erkrankung denken. Nach Schilderung der aus dieser Sicht möglicherweise relevanten Immunphänomene (immunpathologische Studien, immunologische Charakterisierung der beteiligten immunkompetenten Zellen, Lymphocytotoxicitäts-Testung, Mikro- und Makrophagenfunk- tionsprüfungen, HLA-Typisierung) wird auf mögliche pathogenetische Zusammenhänge beim LP eingegangen.

Summary

In lichen ruber planus (LP), a clinically and histologically well defined, aetiopathogenetically uncleari- fied inflammatory disease of skin and/or mucous membranes, exist clinical and laboratory signs for causal or associated immunophenomena. The predominance and close vicinity of T-lymphocytes and macrophages/Langerhans cells in the cutaneous inflammatory infiltrate and the liquefying degeneration of basal keratinocytes, with damage to the basement membrane as well suggest (auto)immune mechanisms involved in the pathogenesis of the disease. After delineation from this point of view the most important immunological findings in LP (immunofluorescence studies, in-situ identification of immunocompetent cell phenotypes, lymphocytotoxicity assay, micro- and macrophage functions, histocompatibility standardization), possible pathogenetic relations are discussed.
