Zusammenfassung
Das Auftreten nächtlicher Atemnot bei Patienten mit obstruktiven Atemwegserkrankungen ist ein sehr charakteristisches Symptom. Unabhängig von der Genese des Krankheitsbildes kommt es bei ea. 60% aller Patienten mit Atemwegsobstruktionen zum Auftreten von Atmnot in den frühen Morgenstunden zwischen 2 und 6 Uhr. Die erste genaue Beschreibung erfolgte bereits im 17. Jahrhundert [7], jedoch werden die pathophysiologischen Grundlagen dieses Symptoms bis heute noch nicht ganz verstanden. Barnes et al. [3] konnten 1980 aufzeigen, daß zirkadiane Schwankungen von Hormonen, Transmittern und Mediatoren pathophysiologisch von Bedeutung sind. Die Ergebnisse weisen eindeutig darauf hin, daß bestimmte Konstellationen biochemisch aktiver Faktoren notwendig sind, um zu bestimmten Tages- oder Nachtzeiten die Bronchien bevorzugt zur Dilatation oder Kontraktion kommen zu lassen.
