Zusammenfassung
Die Behandlung verschiedener Blasentumortypen stellt immer noch eine besondere Herausforderung an den Urologen dar. Die photodynamische Therapie im Anschluß an die intravenöse Verabreichung des Tumorphotosensibilisators Hämatoporphyrin-Derivat (HpD) ist eine neue Behandlungsart oberflächlicher Blasenkarzinome — insbesondere des Carcinoma in situ. Mit diesem Verfahren sind die bisher etablierten Therapieformen zu ergänzen oder in einigen Fällen sogar zu ersetzen. Die photodynamische Therapie bietet ihrem Prinzip nach die Möglichkeit einer vollständigen Erfassung auch von endoskopisch nicht erkennbaren Tumorherden unter Schonung gesunder Blasenwandabschnitte. Grundlage dieser Behandlungsform — eines gewebezerstörenden photodynamischen Prozesses — ist, wie bereits erwähnt, die Verwendung der photosensibilisierenden Substanz Hämatoporphyrin-Derivat (HpD).
