Zusammenfassung
Die unspezifischen chronischen bronchopulmonalen Erkrankungen sind durch polyfaktorielle Ätiologie und Pathogenese gekennzeichnet; neben genetischen Faktoren haben Umweltfaktoren und die Art der Lebensweise eine große, oft entscheidende Bedeutung. Das unterstreicht, wie wichtig es ist, wirkungsvolle Maßnahmen im Sinne der Prävention zu finden. Die Screeninguntersuchungen gehören zur Prävention im Gesundheitswesen. Mit dem Begriff Screening hängt der Begriff der Frühdiagnose eng zusammen, unter welcher wir das Auffinden reversibler Krankheitsanzeichen im vorklinischen, asymptomatischen Stadium verstehen. Die Erkennung der Gesundheitsbeeinträchtigung in dieser Phase schafft in der Regel bessere Voraussetzungen für präventiv-therapeutische Eingriffe als die Diagnose einer klinisch ausgeprägten Erkrankung. Heute mehren sich die Beweise, daß eine so verstandene Frühdiagnostik auch bei unspezifischen chronischen Erkrankungen des Respirationstraktes — wo der Übergang von der Gesundheit zur Krankheit gewöhnlich unauffällig und langfristig vor sich geht — möglich ist (Black 1982). Man schätzt, daß die Entwicklung der chronischen obstruktiven Lungenkrankheit (COPD) 20–30 Jahre dauert, bis es zur Manifestation der Atemnot kommt, des Hauptsymptoms, mit dem der Patient den Arzt aufsucht (Black 1982).
