Summary
From February 1989 until September 1992, 176 cases of acute lower-limb ischemia were treated. The arterial embolism treated by embolectomy with the Fogartycatheter showed good results, depending on the time of intervention. To the contrary, the situation of acute arterial thrombosis had a less favorable prognosis for limb salvage. Therefore, we used an interdisciplinary concept of therapy.
In 24 cases of acute arterial thrombosis (angiography had shown no possibility for a purely surgical treatment) we used, depending on each patient, a combination of intraarterial lysis, angioplasty and operation in the management of these patients.
In 22 of 24 cases, we could save the limb by this procedure. One patient died of cardiovascular failure after major amputation.
Compared to the literature, the combined management with lysis, angioplasty, and operation showed better results than any of these methods used as mono therapy.
