Summary
Of the 1544 patients who underwent aortofemoral bypass grafting during the 25-year period 1965–1989, 210 graft occlusions developed in 172 patients. Using statistical analysis, the efficacy of surgical techniques of repair were compared to three kinds of occlusions: acute occlusions occurred in 20 patients, early occlusions in 36 patients, and late occlusions in 154 patients.
There were different principle causes, including progressive atherosclerosis (n = 156), thrombosis of aneurysms of the suture (n = 10), technical reasons (n = 19), and miscellaneous causes (n = 29).
Surgical techniques included: simple desobliteration with endarterectomy in the region of the distal anastomosis, exclusion of an aneurysm of the suture (n = 154), combined reconstruction with exchange of the graft, repair by cross-over femoro-femo-ral, and femoro-popliteal or -crural bypass, and non-anatomical bypass (n = 56).
The operative mortality rate was 20% for patients with acute occlusions, 0% for early occlusions, and 4% for patients with late occlusions.
The amputation (of a limb) rate was 25%; the rate of early occlusion was 9%, and that for late occlusions was 10%.
Comparative analyses suggested that, following repair of an acute occlusion, the mortality and amputation (leg) rates were significantly higher than for patients with early or late occlusions.
In patients with acute occlusions the surgical repair procedure was associated with a higher rate of complications, due to acute ischemia and repeated operation.
Zusammenfassung
Von den 1544 Patienten, die in einem 25jährigen Zeitraum von 1965 bis 1989 einen aortofemoralen Prothesenbypass erhielten, entwickelten 172 Patienten 210 Prothesenverschlüsse.
Statistisch werden die Ergebnisse der chirurgischen Technik bei den drei Verschluß-arten ausgewertet: Sofortverschlüsse bei 20 Patienten, Frühverschlüsse bei 36 Patienten und Spätverschlüsse bei 154 Patienten.
Die hauptsächlichen Verschlußursachen sind die Progression der Arteriosklerose (n = 156), Nahtaneurysma (n = 10), technische Fehler (n = 19) und verschiedene Ursachen (n = 29).
Chirurgische Methoden zur Beseitigung des Verschlusses: einfache Desobliteration mit Endarteriektomie der distalen Anastomose oder Beseitigung eines Nahtaneurysmas (n = 154), erweiterte Rekonstruktion mit Prothesenaustausch, Anlage eines femorofemoralen Bypass (cross-over), femoropoplitealen oder -kruralen Bypass oder extraanatomischen Bypass (n = 56).
Die postoperative Mortalitätsrate betrug bei Patienten mit Sofortverschlüssen 20%, 0% bei Frühverschlüssen und 4% bei Spätverschlüssen.
Bei Patienten mit Sofortverschlüssen betrug die Amputationsrate 25%, bei Frühverschlüssen 9% und bei Spätverschlüssen 10%.
Die Auswertung der Behandlungsergebnisse zeigt, daß das Amputations- und Mortalitätsrisiko bei Patienten mit Sofortverschlüssen signifikant höher ist als bei den Patienten mit Früh- oder Spätverschlüssen.
Patienten mit Sofortverschlüssen zeigen eine höhere Rate von Komplikationen als Folge der akuten Ischämie und der erneuten Operation.
