Summary
Since 21 years 403 redo operations (15.6%) have to be done after 2654 reconstructions in aorto iliac position. The percentage of patients with arterial occlusive disease had a 3 times higher level (18.3%) compared with patients suffering from aortic aneurysms (5.7%). Early complications have been found in 6.4% as indication for redo surgery; in these cases technical failures or wrong indications were seen at the top of causes. But late complications too showed their origin in technical handicaps in more than 40%. In contrast to other authors recurrent claudication was related to progress of arterial occlusive disease in 0.8% only. The mortality rate after redo surgery didn’t exceed the rate after primary surgery. Complication (41%) and amputation rate (5.8%) show a significant higher level. The quality of the primary operation is responsible for the frequency of redo surgery.
Zusammenfassung
In einem Zeitraum von 21 Jahren wurden bei 2654 aortoiliakalen Wiederherstellungsoperationen 403 Rezidiveingriffe (15,6%) erforderlich. Die Häufigkeit von Rezidiveingriffen war bei Patienten mit AVK mit 18,3% dreimal so hoch wie bei Patienten mit Aortenaneurysmen (5,7%). Die genaue Analyse zeigt in 6,4% der Fälle Frühkomplikationen als Indikation zum Rezidiveingriff; diese waren zumeist auf technische Mängel als pathogenetische Ursache bzw. fehlerhafte Indikationsstellung zurückzuführen. Auch bei Spätkomplikationen konnten formalgenetisch in über 40% der Fälle technische Handikaps ermittelt werden. Entgegen bisheriger Mitteilungen zeigte sich, daß sich eine wiederkehrende Claudicatio intermittens nur in 0,8% eindeutig auf die Progression der arteriellen Verschlußkrankheit zurückführen ließ. Während die Letalität bei Rezidiveingriffen nicht höher lag als bei Primäroperationen, waren eine deutlich höhere Komplikations- (41%) und Amputationsrate (5,8%) zu verzeichnen. Die Qualität des ersten Eingriffs hat einen entscheidenden Einfluß auf die Häufigkeit von Nachoperationen.
