Summary
Endovascular angioplasty has proved its worth in the therapy of aortoilliac occlusive disease; angioplasty opens new possibilities for the surgeon, because it can be used as an adjunct to reconstructive procedures, especially in high-risk patients with multilevel lesions. Our total xperiences includes 45 patients in which vascular reconstruction and intraoperative angioplasty of the iliac region were performed simultaneously.
The procedures included: iliac stenosis, if femoropopliteal bypass-implantation was planned, iliac stenosis and/or femoropopliteal stenosis in combination with common femoral artery reconstruction, angioplasty of stenosed donor iliofemoral segment before femorofemoral bypass-implantation. A stent implantation may improve longterm prognosis of angioplasty. We did not observe any complications. The duration of operation is reduced and the risk is diminished.
Zusammenfassung
Endovaskuläre Angioplastieverfahren haben sich in der Behandlung der aortoiliakalen AVK bewährt. In Kombination mit gefäßchirurgisch-rekonstruktiven Verfahren erweitern sie das therapeutische Spektrum, insbesondere bei Hochrisiko-Patienten mit einer Mehretagenproblematik. Aufgrund der Erfahrung bei 45 Patienten hat sich für die transluminale Katheterbehandlung folgendes Indikationsspektrum herauskristallisiert: Intraoperative Dehnungsbehandlung einer Iliakastenose bei geplantem femoropoplitealem Bypass, Angioplastie vor- und nachgeschalteter Stenosen bei Femoralisgabelrekonstruktion, Dilatation einer stenosierten Spenderarterie bei femorofemoraler Bypass-Implantation. Die primäre Stent-Implantation kann möglicherweise den Langzeiteffekt der Angioplastie verbessern. Katheterspezifische Komplikationen wurden nicht beobachtet. Die Katheterbehandlung verkürzt die Operationszeit und minimiert das operative Risiko einer Zweietagenkorrektur.
