Die asymptomatische Karotisstenose — pro Operation

Zusammenfassung

Die Inzidenz zerebrovaskulärer Erkrankungen hat im letzten Dezenium deutlich zugenommen. Sie rangieren derzeit mit 15% aller Todesursachen hinter Malignomen und kardiovaskulären Erkrankungen an dritter Stelle. Wie aus den meisten Sektionsstatistiken hervorgeht, werden bis zu 60% der ischämisch bedingten Schlaganfälle durch oblitierende Prozesse der extrakraniellen A. carotis interna hervorgerufen [10]. Für die BRD errechnet sich damit eine karotisbedingte Mortalitätsrate von mindestens 20 000–30 000 pro Jahr Damit ist zu überlegen, ob aus dieser Sicht eine operative Schlaganfallprophylaxe bei asymptomatischen Karotisstenosen angezeigt ist. Zur Beantwortung dieser Frage wurden Indikation und Behandlungsergebnisse analysiert, und es wurde zur Wertigkeit einer prophylaktischen Operation nach Durchsicht der Literatur Stellung genommen.
