Zusammenfassung
Eine Beeinträchtigung der Leistungsfähigkeit durch blutdrucksenkende Medikamente ist von wesentlicher praktischer Bedeutung, da einerseits viele Hypertoniker noch jung sind und andererseits zahlreiche Hypertoniker ein regelmäßiges körperliches Training durchführen. Je geringer die Leistungsbeeinflussung ist, um so weniger wird die Patienten-Compliance beeinträchtigt sein. Das ideale Antihypertensivum beim sporttreibenden Hypertoniker sollte den Blutdruck sowohl in Ruhe als auch unter Belastung ausreichend senken und weder zu einer Beeinflussung der Leistungsfähigkeit noch zu einer Behinderung der Ausbildung von Trainingseffekten führen. Die Deutsche Hochdruckliga empfiehlt, mit einer Monotherapie eines Betablockers oder Kalziumantagonisten oder Diuretikums oder ACE-Hemmers zu beginnen. In der zweiten bzw. dritten Stufe werden Zweier- oder Dreierkombinationen vorgeschlagen. Im folgenden wird die antihypertensive Therapie des sporttreibenden Hypertonikers in der Reihenfolge der genannten antihypertensiven Substanzen unter besonderer Berücksichtigung der körperlichen Leistungsfähigkeit besprochen.
