Zusammenfassung
Die ambulante Blutdrucklangzeitmessung (ABDM) ermöglicht heute, über 24–48 Stunden Blutdruck und Herzfrequenz unter Alltagsbedingungen zu messen (Tabelle 1). Die direkte 24-Stunden-Blutdruckmessung stellt ein kontinuierliches Meßverfahren dar, ermöglicht eine sog. beat-to-beat-Analyse (d. h. die Registrierung und Auswertung von über 120.000 Meßpunkten pro Stunde). Wegen des — wenn auch geringen — Risikos einer Blutung, einer Infektion oder einer Nervenschädigung im Bereich der A. brachialis (oder radialis) steht dieses Meßverfahren (30) ausschließlich für klinische Forschungszwecke zur Verfügung, ist jedoch für den routinemäßigen Einsatz nicht geeignet. Dagegen ermöglicht heute die indirekte 24-Stunden-Blutdrucklangzeitmessung diskontinuierlich (d. h. auskultatorisch, oszillometrisch) oder kontinuierlich (phlethysmographisch) eine zuverlässige ambulante wiederholte Registrierung des Blutdrucktagesprofils (d. h. die Registrierung von 70–200 Meßpunkten pro 24 Stunden), eine neue Methode, die in der Diagnostik und zur Therapiebeurteilung herangezogen werden kann (21).
