Zusammenfassung
Die Zerstörung von gesundem Knorpel und Knochen im Verlauf der rheumatoiden Arthritis ist durch ein besonderes Gewebe gekennzeichnet, das als Pannus bezeichnet wird. Dieser Pannus ähnelt histologisch einem Granulationsgewebe, das entweder dem Knorpel aufliegt und dort zur Erosion führt oder als zerstörendes Gewebe vom Gelenkrezessus in den subchondralen Markraum vordringt. Das Pannusgewebe ist histologisch und histochemisch eingehend charakterisiert worden [2, 4, 7, 9, 10, 12]. Dabei wurde ein zellreicher und ein zellarmer Pannus unterschieden. Im zellreichen Pannus und an der dazugehörigen Pannus-Knorpel-Grenze dominierten Makrophagen und Fibroblasten, es wurden jedoch auch Mastzellen, Plasmazellen, Granulozyten und Lymphozyten gefunden. Der zellarme Pannus wurde als ein fibröses Gewebe beschrieben, in dem Entzündungszellen wie z. B. Makrophagen selten angetroffen wurden. Konventionelle histologische und histochemische Untersuchungen erlauben jedoch nicht immer eine eindeutige Bestimmung des Zelltyps. Dies mag eine der Ursachen dafür sein, daß die Häufigkeit von Granulozyten im Pannus sehr unterschiedlich beurteilt worden ist [9, 10] und Makrophagen von Fibroblasten nicht immer sicher zu unterscheiden waren. Mononukleäre Zellen, wie z. B. T-Lymphozyten, B-Lymphozyten und Natural-killer (NK)-Zellen können mit konventionellen Methoden ebenfalls nicht differenziert werden.
