Zusammenfassung
Grundlage der hämolytischen Fetalerkrankung ist das Vorhandensein mütterlicher Alloantikörper gegen Erythrozytenmerkmale des Kindes. Am häufigsten und wichtigsten sind Blutgruppenunverträglichkeiten im Rh-System, speziell gegen den Faktor D. Vor Einführung einer konsequenten Rh-Sensibilisierungsprophylaxe, um die sich Schneider (52, 53, 54, siehe auch den Beitrag im vorliegenden Band) in der Bundesrepublik besonders verdient gemacht hat, trat eine Rhesuserythroblastose in 0,2–0,6% aller Schwangerschaften bis etwa 1970 auf (40). Der absolute Rückgang von Schwangerschaften mit einer Sensibilisierung wegen des Rhesusfaktor D führte zu einer relativen Zunahme von Immunisierungen gegen andere Antigene des Rh-Systems sowie andere Blutgruppensysteme wie Kell, Duffy, Kidd und MNSs (61, 62). Im eigenen Kollektiv mit 234 sensibilisierten Schwangeren in den Jahren 1967 bis 1988 fanden sich im Zeitraum 1967–1974 in 98,4% Anti-D-Antikörper, 1975–1982 in 89,7% und 1983–1988 nur noch 71,7% (Tabelle 1). Während im ersten Zeitraum etwa 10% der Sensibilisierungen auf eine Bluttransfusion zurückzuführen waren, waren es im mittleren Zeitabschnitt 18,6% und in den Jahren 1983 bis 1988 43,6% (Tabelle 2).
