Zusammenfassung
Die Diagnostik und Therapie myokardialer Ischämiezustände stellt angesichts der hohen Prävalenz der koronaren Herzkrankheit eine besondere Herausforderung dar. Dabei zeigte sich in den letzten Jahren, daß das klinische Leitsymptom Angina pectoris bei einem Teil der Patienten mit gesicherter koronarer Herzkrankheit teilweise oder sogar vollständig fehlen kann. In der Framingham-Studie fand sich bei 708 der insgesamt 5127 beobachteten Personen im routinemäßig angefertigten EKG zufällig der Befund eines abgelaufenen Myokardinfarktes (1). Anamnestisch berichteten 15% dieser Personen darüber, zu keinem früheren Zeitpunkt Herzbeschwerden gehabt zu haben. Zahlreiche weitere Untersuchungen in jüngster Zeit bestätigen, daß stumme Myokardischämien wesentlich häufiger auftreten, als dies bislang angenommen wurde (2–6). Dem kommt um so mehr Bedeutung zu, als neuere Ergebnisse auf einen kumulativen Effekt von kurz dauernden Ischämieepisoden zur Ausbildung größerer Myokardnekrosen (7) und elektrischer Instabilitäten hinweisen (8). Im folgenden soll auf die qualitativen und quantitativen Aspekte stummer Myokardischämien im Vergleich zu symptomatischen Episoden eingegangen werden. Andererseits wird auch auf die technischen und methodischen Probleme hingewiesen sowie erste Therapieergebnisse bei stummer Myokardischämie aufgezeigt.
