Indikation zur Myokardrevaskularisation bei koronarer Herzkrankheit — Standortbestimmung 1985

Zusammenfassung

Fortschritte in der medikamentösen Behandlung der koronaren Herzkrankheit und bei der perkutanen transluminalen Koronardilatation haben die Indikation zur chir-urgischen Myokardrevaskularisation mittels des aortokoronaren Bypass in Richtung auf fortgeschrittenere Stadien der koronaren Herzkrankheit verschoben. Die optimalen Operationskandidaten mit proximal stenosierten, distal der Stenose je-doch weitlumigen Koronargefäßen und gut erhaltener Ventrikelfunktion werden mehr und mehr abgelöst durch Patienten mit diffus veränderten Gefäßen und ver-narbtem Myokard, die für eine perkutane transluminale Dilatation ungeeignet sind und auf eine medikamentöse Therapie nicht ausreichend ansprechen.
