Zusammenfassung
Bekanntlich ist das Herz weitgehend auf den aeroben Stoffwechsel, also auf die ATP-Produktion durch die oxidative Phosphorylierung in den Mitochon- drien angewiesen. Bei Ischämie sistiert die Anlieferung von Sauerstoff und von Substraten, der Abtransport von Stoffwechselendprodukten ist verhindert. Da die maximale Rate der ATP-Produktion durch die anaerobe Glykolyse sehr gering ist (11, 21), kommt es zu einer Imbalance zwischen Energieproduktion und -utilisation. Bereits innerhalb der ersten Minuten nach Eintritt der Ischämie wird der Gehalt von Kreatinphosphat und von ATP vermindert, das anorganische Phosphat steigt an. Der steilste Abfall ist bereits innerhalb der ersten 30 Sekunden beim Kreatinphosphat zu beobachten. Die Verminderung des ATP-Spiegels erfolgt mit etwas geringerer Geschwindigkeit. Beim ATP-Abbau über ADP und AMP kommt es zur Entstehung von Adenosin, Inosin, Hypoxanthin, Xanthin und schließlich Harnsäure (3, 5, Abb. 1, gestrichelte Pfeile). Da die dephosphorylierten Abbauprodukte die Zellmembran permeieren, entsteht ein echter Verlust, wenn während der Reperfusion die Nukleoside und Basen ausgewaschen werden.
