Zusammenfassung
Die Tätigkeit des Herzens ist auf die spontane Erregungsbildung im Sinusknoten, die Fortleitung der Erregung und die Koppelung von Erregung und Kontraktion zurückzuführen (13). Diese primär im Herzen selbst gesteuerten Vorgänge werden durch neurohumorale Faktoren beeinflußt (7). Im Vordergrund steht hierbei das autonome Nervensystem, das mit seinen beiden Anteilen, dem Sympathicus und dem Parasympathieus, erregende bzw. dämpfende Einflüsse auf das Herz vermittelt. Die parasympathische Versorgung des Herzens erfolgt über die nervi vagi und betrifft bevorzugt den Sinus- und AV- Knoten, die Vorhöfe sowie die initialen Anteile des Reizleitungssystems, während die Ventrikelmuskulatur weniger innerviert ist (14). Die kardialen Wirkungen des Parasympathieus werden durch die Freisetzung von Acetylcholin vermittelt, das postsynaptisch die Erregung von Muskarinrezeptoren hervorruft (7). Im vorliegenden Beitrag soll insbesondere der Mechanismus der durch die Rezeptoraktivierung vermittelten postsynaptischen Wirkungen am Vorhof näher erläutert werden.
