Zusammenfassung

Die akute respiratorische Insuffizienz wird definiert als akuter Abfall des arteriellen Sauerstoff-partialdrucks unter altersentsprechende Normwerte, unter Berücksichtigung des Barometerdruckes und bei Abwesenheit eines intrakardialen Rechts-Links-Shunts (12). Verbesserte patho-physiologische Kenntnisse haben zu neuem Verständnis der verschiedenen Krankheitsbilder geführt, die man unter dem Begriff akute respiratorische Insuffizienz subsumieren kann. Damit wird eine Klassifikation ermöglicht, die die intrathorakalen Gasvolumina bzw. die funktionelle Residualkapazität in den Mittelpunkt der Unterscheidung stellt (21). Es wird differenziert zwischen Formen der akuten respiratorischen Insuffizienz mit erhöhtem oder erniedrigtem Lungenvolumen (Tabelle 1).
