Zusammenfassung
Robertson und Mitarbeiter haben bereits 1969 (6) gezeigt, daß Kalziumsteinpatienten mit kurzfristigem Rezidiv unter identischen Diätbedingungen größere Kalziumoxalat-kristalle ausscheiden als gesunde Kontrollen und daß sich diese Kristalle bei den Patienten zu bis zu 200μm großen Aggregaten zusammenlagern. Die erhöhte Kristall-wachstums- und Aggregationstendenz im Patientenurin ist einerseits auf eine gesteigerte Übersattigung an Kalziumoxalat und andererseits auf eine verminderte Inhibitoraktivität zurückzuführen (7), wobei einem Mißverhältnis zwischen beiden Faktoren mehr Bedeutung zukommt als einer Einzelstörung. So konnten die Autoren ihre 8 Steinpatienten und 8 gesunden Kontrollen am besten anhand eines Sättigungs-inhibitionsindexes unterscheiden. Dieser Index wurde indirekt aufgrund einer Computerberechnung der Urinsättigung und eines Hemmtestes für Kristallwachstum und Aggregation bei starker Urinverdünnung (1%) ermittelt.
