Mitral Valve Function and Cardiac Dimensions During Ventricular (VVI) and Sequential Atrio-Ventricular (DVI) Pacing. Echocardiographic Study

Summary

We studied 16 patients with implanted multiprogrammable DVI pacemakers, utilizing M-mode and two dimensional (2D) echocardiography, during DVI and VVI pacing.

Left ventricular dimensions in both systole and diastole were significantly larger during DVI pacing compared to VVI (p < 0.001). Conversely left atrial (LA) dimensions were significantly smaller during DVI pacing compared to VVI (p < 0.001). The duration of “effective” mitral valve opening (MVO) was significantly longer during DVI pacing (p < 0.001). All patients demonstrated septal dyssynergy during right ventricular pacing, regardless of the mode. Atrial competition was a frequent finding during DVI pacing and this correlated with an ineffective atrial kick demonstrated echocardiographically.
