Zusammenfassung
Ein indirektes Kriterium für linksventrikuläre Hypertrophie (LVH) ist eine Elevation der Potentiale unter die Horizontalebene des dem korrigierten, orthogonalen Ableitungssystem zugrunde liegenden Koordinatensystems (18, 23). Die damit verbundene Elevation des räumlichen QRS- Hauptvektors überschreitet in der Regel selbst bei schwerer Druckhypertrophie nicht diese Horizontalebene. Obwohl schon 1917 Rothberger und Winterberg (31) auffällige Abweichungen der elektrischen Herzachse nach experimenteller Unterbrechung einer der beiden Aufzweigungen des linken Tawara-Schenkels beschrieben, galt der überdrehte Linkstyp (ü.L.T.) noch in der Folgezeit als indirektes Kriterium für LVH (16). Man nahm als Ursache Rotation eines horizontal gelegenen Herzens um die Längsachse im Gegenuhrzeigersinn an, wodurch die insbesondere bei LVH vergrößerten posterioren Potentiale eine räumlich linkssuperiore Ausrichtung erfahren würden. Grant fand keine Beziehung zwischen anatomischer Längsachse und frontaler elektrischer Achse, konnte diesem Ekg-Typ keine auffällige Herzrotation zugrunde legen und fand keine Abhängigkeit von konstitutionellen Besonderheiten (13, 14, 15). Die klinische Erfahrung lehrt, daß die schwere LVH diesen Ekg-Typ nur gelegentlich aufweist, während man ihn bei Patienten mit verschiedenartigen Herzerkrankungen und auch bei anscheinend Herzgesunden nicht selten sieht (1, 3, 4, 5, 6, 7, 9, 10, 12, 17, 21, 22, 26, 29, 34).
