Zusammenfassung
In neuerer Zeit ist mehrfach iiber die erfolgreicbe Anwendung von Glucagon bei akuter und chronischer Herzinsuffizienz berichtet worden (1, 3, 7, 8, 14, 15, 20, 22). Der genaue Wirkungsmechanismus ist bisher nicht bekannt. In metabolischer Hinsicht entspricht die Glucagon-Wirkung am Herzen weitgehend der der Katecholamine. Sowohl Glucagon als auch Adrenalin aktivieren an der intakten Herzmuskelzelle das strukturgebundene Adenyl-Cyclase-System an einem noch hypothetischen Angriffsort der Zellmembran, das mit dem Beta-Rezeptor nicht identisch ist. Die vermehrte Bildung von zyklischem 3’5’-AMP, die Aktivierung der Phosphorylase und der positiv inotrope Effekt sind zeitlich eng miteinander verknüpft. Beta- Rezeptorenblocker (Pronethalol) hemmen den Adrenalineffekt, nicht dagegen die Wirkung von Glucagon auf dieses System, was auf verschiedenartige Rezeptoren oder verschieden konfigurierte Adenyl-Cyclasen hinweist (Untersuchungen an perfundierten isolierten Rattenherzen: Mayer und Mitarb. [13]). In hämodynamischer und elektrophysiologischer Hinsicht bestehen zwischen Glucagon und den Katecholaminen wesentliche Unterschiede (5, 10, 12, 18, 21): Glucagon ist im Tierexperiment auch nach Vorbehandlung mit Reserpin positiv inotrop wirksam.
