Zusammenfassung
Im Gleichschritt mit wachsenden Erkenntnissen im Fach der Hämostaseolo- gie weiten sich ebenfalls die Möglichkeiten der perioperativen Bedsidegerin- nungsevaluierung. Neben den plasmatischen Standardgerinnungstests, wie Quick-Test, partielle Thromboplastinzeit, Thrombinzeit, Fibrinogenspiegel und Antithrombin-III-Aktivität, welche neuerdings auch in Cartridge-Form als Bedsidetest erhältlich sind, stellen einige globale Vollblutmeßmethoden gegenwärtig einen Eckpfeiler des perioperativen Gerinnungsmanagements dar. Im Gegensatz zu den vorhin genannten Gerinnungsmeßverfahren ermöglichen die z.Z. erhältlichen mechanischen Vollblutmeßmethoden ob ihres leichten Handlings, der leichten Interpretierbarkeit und des geringen Platzaufwandes eine Positionierung direkt am Patienten und damit ein On-line- Monitoring. Diese Systeme berücksichtigen nicht nur eine Gerinnungskomponente isoliert, sondern auch die Interaktion zwischen plasmatischem und zellulärem System. Mit Ausnahme der „activated clotting time“ (ACT), welche nur die initiale Gerinnselbildung erfaßt, geben die auf dem Prinzip der Viskoelastizitätsmessung basierenden Systeme wie die Thrombelastographie (Fa. Haemoscope, Illinois, USA), die Resonanzthrombelastographie (Fa. Amelung, Bad Salzuflen, BRD) und der Sonoclot-Analyser (Fa. Sienco, Morrison, Colorado, USA) eine Übersicht über Thrombusbildung, Stabilität und Lyse.
