Zusammenfassung
Trauma und große operative Eingriffe führen, ebenso wie septische Zustandsbilder zu einer Störung der Homöostase des Immunsystems. Dabei treten alternierend hyperinflammatorische Phasen und ein Übergewicht von anti-inflammatorischen oder immunsupprimierenden Potentialen auf. Ein Übergewicht von anti-inflammotorischen Potentialen könnte möglicherweise die Entwicklung infektiöser Komplikationen begünstigen und wurde mit dem Begriff der Immunparalyse belegt [1]. Dieser Zustand ist u.a. durch eine reduzierte Synthesefähigkeit der Zytokine TNFα, IL-1 oder IL-6 von mononukleären Zellen gekennzeichnet [2–4]. Durch Bestimmung der Zytokinfreisetzung von Vollblut nach Endotoxinstimulation ex vivo sollte es möglich sein, den aktuellen Immunstatus des Patienten abzuschätzen.
