Zusammenfassung
Den meisten Patienten mit Rektumkarzinom kann heute dank moderner chirurgischer Technik eine sphinktererhaltende Resektion angeboten werden. Es konnte in der Vergangenheit gezeigt werden, daß Stuhldrang und Stuhlkontinenz nach einer anterioren Rektumresektion von der Länge des verbliebenen Rektumstumpfes abhängen. Eine akzeptable anorektale Funktion bleibt dann erhalten, wenn eine kritische Distanz von 7 cm zwischen Analkanalrand und Anastomose nicht unterschritten wird [1]. Ist die totale mesorektale Exzision das Standardverfahren um lokale Radikalität bei Tumoren im distalen und mittleren Rektumbereich zu erreichen, dann ist häufig die Anlage eines sehr tiefen Anastomose notwendig. Bei solchen Eingriffen wird das Rektum in Höhe der Levatorenebene abgesetzt und die Anastomose am Oberrand des Analkanals oder im Bereich des distalen Rektums angelegt [2].
