Entwicklungsgeschichte der ileoanalen Anastomose — 20 Jahre J-Pouch

Zusammenfassung

In diesem Jahr begehen wir den hundertsten Jahrestag der ersten erfolgreichen totalen Gastrektomie durch Schlatter im Jahre 1897. Seither gelang der gastrointestinalen Chirurgie eine Reihe von Durchbrüchen in bezug auf rekonstruktive Verfahren. Bis vor 20 Jahren blieb die funktionelle Rekonstruktion nach einer Proktokolektomie eines der wenigen großen ungelösten Probleme. Zur erfolgreichen Behandlung einer Colitis ulcerosa (CU) und einer familiären Polyposis (FAP) ist die komplette Entfernung der colorektalen Mukosa notwendig. Diese beiden Erkrankungen sind im Grunde gutartig und die Patienten sind oft jung. Die chirurgische Entwicklung hat die Aufgabe diesen Patienten eine immer bessere postoperative Lebensqualität zu ermöglichen.
