Zusammenfassung
Die restaurative Proktokolektomie mit den verschiedenen Typen des pelvinen Ileum-reservoirs findet heute in der elektiven Chirurgie der Colitis ulcerosa und der familiären Polyposis weite Anwendung. Durch einen Ileum-Pouch steigt die Kapazität und Compliance des distalen Ileums, die Peristaltik wird abgeschwächt. Mit Hilfe eines solchen Ileumreservoirs lassen sich wesentlich bessere klinische Ergebnisse erzielen als durch eine direkte ileoanale Anastomose [2]. Die ileoanale Anastomose (IAA) ohne Reservoir liefert nur unbefriedigende klinische Ergebnisse. Durch die zu geringe Speicherkapazität des terminalen Ileums und durch Hochdruckwellen im Ileum resultiert eine excessiv erhöhte Stuhlfrequenz und manchmal sogar eine Inkontinenz.
