Zusammenfassung
Die Aufrechterhaltung eines stabilen Verhältnisses von intra- und extrazellulären Volumina zählt zu den primären Aufgaben physiologischer Regulationsmechanismen. Diese beziehen sich primär auf den Haushalt von Wasser und Natrium durch zentralnervöse Beeinflussung der systemischen Aufnahme (Durstgefühl) sowie durch die renale Ausscheidung mittels aktiver Exkretions- und Reabsorptionswege. Zentralnervöse Regulatoren wie das Durstgefühl sind beim sedierten und kontrolliert beatmeten Patienten meist reduziert bzw. ausgeschaltet, sodaß die Nieren als wichtigste Determinante für die Bilanz von Wasser und Elektrolyten zu betrachten sind. Veränderungen des Flüssigkeitshaushalts werden bei beatmeten Patienten regelmäßig beobachtet. Dies betrifft besonders Langzeitbeatmete, z. B. Patienten mit akutem Lungenversagen (engl.: „acute respiratory distress syndrome“, ARDS; „acute lung injury“, ALI), und kann für den intensivmedizinischen Verlauf erhebliche Folgen haben. Verschiedene pathophysiologische Wege werden hierfür diskutiert: Hierzu zählen einerseits direkte intrapulmonale Effekte durch die erkrankte Lunge, meist infolge von vaskulären Schäden, andererseits indirekte Effekte durch die beim ALI und ARDS notwendigen Beatmungsdrücke auf Hämodynamik, sympathische Aktivität sowie auf die neuroendokrinen und renalen Regulationsmechanismen (Krebs u. Kaczmarczyk 1994).
