Zusammenfassung
Die Mehrzahl der Melanompatienten (> 90 %) weist zu Beginn der Erkrankung einen Primärtumor ohne erkennbare Metastasierung auf. Abhängig von der Dicke des Tumors kommt es bei einem Teil der Patienten im weiteren Verlauf zu einer lymphogenen sowie hämatogenen Metastasierung. Bei Patienten mit dicken Primärtumoren, Lokalrezidiven, In-transit Metastasen sowie regionären Lymphknotenmetastasen ist die Prognose durch eine drohende hämatogene Fernmetastasierung drastisch eingeschränkt. Zur Abwendung einer Tumorprogression werden daher bei diesen Patienten seit vielen Jahren adjuvante Therapieverfahren im Rahmen kontrollierter Studien durchgeführt. Zum Zeitpunkt der Einleitung dieser prophylaktischen Behandlungsverfahren ist der Patient klinisch und „labortechnisch“ tumorfrei. Das Ziel aller adjuvanten Therapiekonzepte ist die Elimination möglichst aller Mikrometastasen. Üblicherweise werden Formen der postoperativen adjuvanten Therapie gewählt, die sich bei fortgeschrittenem Tumorleiden als wirksam erwiesen haben.
