Zusammenfassung
Bei der kombinierten Katarakt- und Glaukomoperation besteht aufgrund der ausgeprägten Entzündungsreaktion das Risiko einer Filterkisseninsuffizienz. In einer prospektiven randomisierten Studie untersuchten wir die Abhängigkeit der Filterkissenfunktion Von der Größe des operativen Zugangs. Bei jeweils 25 Patienten führten wir eine gedeckte Goniotrepanation mit simultaner Phakoemulsifikation und Implantation einer Silikonlinse bzw. Implantation einer 6-mm-PMMA-HKL durch. Nach 6 Monaten fanden wir in der Gruppe der Patienten mit Silikonlinse bei 20 ein suffizientes Filterkissen, während bei den Patienten mit PMMA-Linse lediglich 14 ein Filterkissen aufwiesen (p < 0,025). Die Anzahl der Patienten, die eine zusätzliche antiglaukomatöse Therapie benötigten, betrug nach Silikonlinsenimplantation 4, in der Vergleichsgruppe 14 Patienten (p < 0,01).

Summary
In combined cataract and glaucoma surgery the risk for insufficiency of the filtering bleb is higher than in simple glaucoma surgery. In a prospective, randomized study we investigated the influence of the incision technique on the outcome of the filtration. In 25 patients we performed goniotrephination covered with a scleral flap, phacoemulsification and implantation of a folded silicone lens beneath the base of the scleral flap. In the control group we implanted a 6-mm PMMA. Six months later 20 of the patients with silicone lens still showed a filtering bleb, in contrast to only 14 of the patients with PMMA lens (p < 0.025). Four patients of the silicone lens group received additional glaucoma treatment, in contrast to 14 of 25 patients with PMMA lens (p < 0.01).
