Zusammenfassung
Dieses Review soll in kurzer Form einige Aspekte der Nachstarreduzierung durch die Veränderung der kataraktchirurgischen Techniken während der letzten 15 Jahre beispielhaft aufzeigen. Hierzu wurden Befunde von enukleierten Autopsieaugen, die kataraktoperiert und mit Hinterkammerlinsen versorgt wurden, analysiert. Die Präparate sind dem „Center for IOL Research“ über einen Zeitraum von 15 Jahren zur pathologischen Begutachtung zugesandt worden. Die Beobachtungen basieren auf einer Datenbank mit etwa 12000 Präparaten. Darunter befinden sich auch ca. 250 explantierte moderne Faltlinsen und ca. 120 Autopsieaugen mit Faltlinsen. Die Entwicklung in der Kataraktchirurgie zur sog. „Capsular Surgery“ mit den damit verbundenen modernen OP-Techniken (Kapsulorhexis, Hydrodissektion, Phakoemulsifikation, Cortical-clean-up, Kapselsackfixation) führte zu einer signifikanten Verbesserung der Intraokularlinsenfixation und Zentrierung. Die Entwicklung eines Soemmerring’s-Ringes zeigt in Präparaten, die mit o. g. Techniken operiert wurden, ein deutlich geringeres Ausmaß, damit einhergehend scheint auch die zentrale Nachstarentwicklung verringert bzw. zeitlich erheblich verzögert. Diese Entwicklung scheint in dem bisher vorliegenden Autopsiematerial unabhängig vom HKL-Design oder -material und eher Ausdruck des sorgfältigen chirurgischen Vorgehens zu sein. Für moderne Faltlinsen liegen erst vorläufige Ergebnisse vor, die jedoch auch im o. g. Trend liegen.
Summary
This paper reviews some aspects of factors influencing and reducing posterior capsule opacification (PCO) with special reference to changes and trends in cataract surgical techniques of the past 15 years. The findings of autopsy eyes with IOLs and explanted IOLs that have been submitted to the Center for IOL Research over the past decade have been gathered in a large database. This database now consists of 12,000 specimens, including approximately 250 explanted modern foldable IOLs and 120 enucleated autopsy eyes with foldables. The development of cataract surgery to so-called capsular surgery, comprising modern surgical techniques such as continous curvilinear capsulorhexis, hydrodissection, phacoemulsification, cortical clean-up, and in-the-bag fixation, lead to significantly better IOL fixation and centration. Autopsy eyes operated in the 1990s with capsular surgical techniques showed less Soemmerring’s ring formation and consequently less or delayed PCO development. This was independent from IOL material or design in the autopsy material reviewed so far and more related to careful use of modern capsular surgical techniques, especially thorough cortical clean-up. Preliminary observations in specimens with foldable IOLs seem to follow this trend.
