Zusammenfassung
Das Ulcus ventriculi und das Ulcus duodeni sind flächenhafte Schleimhautdefekte im Magen und Duodenum, die bis in die Submukosa reichen. Es handelt sich wahrscheinlich um nosologisch unterschiedliche Erkrankungen mit unterschiedlicher Epidemiologie, Spontanverlauf und Pathophysiologie. Da die Ätiologie beider Erkrankungen bislang ungeklärt ist, sie jedoch durch einen chronisch rezidivierenden Verlauf gekennzeichnet sind, benachbarte Organe betreffen und für ihre Entwicklung die aggressive Einwirkung von Salzsäure und Pepsin auf die Schleimhaut eine „conditio sine qua non“ darstellt, kann man sie unter dem Begriff der Ulkuskrankheit zusammenfassen. Die akute Streßläsion des oberen Gastrointestinaltraktes, das sog. Streßulkus, ist eine andere Erkrankung, die nicht unter diesen Oberbegriff der Ulkuskrankheit paßt (s.6.3). Die Streßläsion entsteht immer nach einem physiologischen Streß mit den klinischen Zeichen eines Schocks, beispielsweise nach chirurgischen Eingriffen, Hirntraumata, Verbrennungen oder Sepsis. Unglücklicherweise haben in der Internationalen Klassifikation der Erkrankungen (ICD-Code) die Ulkuskrankheit und die Streßläsion bislang noch keine getrennte Einteilung erfahren. Manchmal wird unter Streßulkus auch ein Ulkus verstanden, für dessen Entstehung ein psychischer Streß verantwortlich gemacht wird. Dieser Ulkustyp gehört unter dem Oberbegriff Ulkuskrankheit und keinesfalls in die Gruppe er „chirurgischen“ Streßläsionen.
