Zusammenfassung
Zu Beginn der zementverankerten Hüftendoprothese fehlten Erfahrungen mit Spätkomplikationen. Art und Häufigkeit waren wegen der ausstehenden Langzeitverläufe noch nicht abzuschätzen. Operationstechnik, Prothesengestaltung, Indikation und Infektionsverhütung mußten schrittweise erlernt werden. Als häufigste Versagensursache kennen wir heute die mechanische Lockerung. Nach mehr als 10 Jahren realer Belastungszeit rechnen wir z. Z. etwa ab dem 7. postoperativen Jahr mit jährlichen Ausfallraten von 0,5–1%. Die Infektion ist nach wie vor die ernsteste Komplikation. Nach über 10 Jahren beträgt die Rate der Früh-und Spätinfektionen etwa 2%. Dabei hat sich gezeigt, daß die zunehmende Erfahrung des Operateurs mit dem Prothesensystem ein ausschlaggebender Faktor für die Haltbarkeit ist [3, 10].
