Zusammenfassung
Die Versorgung des exokrinen Anteiles und die Abstoßungsdiagnostik stellen nach wie vor die Hauptprobleme im Zusammenhang mit der Pankreastransplantation dar. Das Monitoring der endokrinen Transplantatfunktion hat sich als nicht zielführend erwiesen, da ein pathologischer Kohlehydratstoffwechsel erst nach Untergang von über 90% der Inselzellmasse nachweisbar wird. Um das Pankreastransplantat nicht durch eine synchrone Gangokklusion zusätzlich zu den Manipulationen der Transplantation zu belasten und um die exokrine Pankreasfunktion für die Abstossungsdiagnostik nützen zu können, wurde eine Methode entwickelt, bei der wir den Pankreassaft bis zur Stabilisierung der Transplantatfunktion über ein Drain nach außen ableiten und den Gang erst sekundär okkludieren..
