Zusammenfassung
Forskolin ist eine neue Substanz mit positiv inotroper sowie vasodilatierender Eigenschaft, die als Diterpenderivat aus Coleus forskolii (Abb. 1) keine sturkturelle Ähnlichkeit mit Glykosiden, Katecholaminen, Xanthinen, dem Aminobipyridinderivat Amrinon oder dem Phenylimidazophyridinderivat Sulmazol aufweist. Untersuchungen am isolierten Herzgewebe belegen, daß Forskolin seine Wirkung über direkte Aktivierung der membranständigen Adenylatzyklase und einer zytoplasmatischen cAMP-abhängigen Proteinkinase entwickelt [12, 14]. Die Kombination von Ino-tropiezuwachs und Vasodilatation läßt erwarten, daß der inotropiebedingte myo-kardiale Energiemehrbedarf durch die Begleitwirkung auf Vor- und Nachlast quantitativ kompensiert werden kann. Ferner verspricht die Eigenschaft, daß die Forskolinwirkung nicht an die β-Rezeptoren gebunden ist, sondern durch direkte Einwirkung auf die katalytische Subeinheit der Adenylatzyklase vermittelt wird, therapeutisch nutzbare Vorteile gegenüber den Katecholaminen. So konnten Bristow et al. [3] an explantierten, gegenüber Katecholaminen subsensitiven humanen Herzen durch Forskolin weitere Inotropiereserven mobilisieren.
