Zusammenfassung
Bei der Auswahl der für den jeweiligen Patienten geeigneten Pharmaka aus der Palette der verfügbaren Anästhesieverfahren ist der Aspekt der Sicherheit von herausragender Bedeutung. Dieser wird vorrangig von der Erfahrung im Umgang mit den in Betracht kommenden Substanzen und von deren Einfluß auf die vitalen Funktionen determiniert. Die Kenntnis günstiger und unerwünschter pharmakodynamischer Effekte von Anästhetika auf die wichtigen physiologischen Funktionssysteme ist deshalb eine unerläßliche Voraussetzung ihrer sicheren Anwendung. Der Einfluß von Anästhetika auf Lunge und Atmung muß v. a. bei Patienten mit Erkrankungen, die Ventilation und Gasaustausch betreffen, berücksichtigt werden. Auch der operative Eingriff als solcher und spezielle anästhesiologische Techniken (z. B. Lungenresektionen in Seitenlage und mit „one-lung-ventilation“) können die pulmonale Funktion erheblich alterieren und müssen deshalb in den Entscheidungsprozeß über die geeigneten Anästhetika eingehen.
