Zusammenfassung
Von Imipramin ist bekannt, daß es mit hoher Affinität an eine regulatorische Untereinheit des Transportproteins bindet, das Serotonin aus dem Blut in die Thrombozyten transportiert (Brunello et al. 1982; Grob et al. 1984; Sette et al. 1981; Talvenheimo et al. 1978). Dieser Mechanismus wird auch als Modell für den Rücktransport des Serotonins aus dem synaptischen Spalt in die Präsynapse von Neuronen im ZNS angesehen (Langer u. Briley 1981). Von Schlafentzug (SE) ist zu vermuten, daß er serotonerge Neurone beeinflußt. Es gibt jedenfalls Befunde, die zeigen, daß ein positives Ansprechen auf SE mit späterem Ansprechen auf das vorwiegend serotonerge Neurone modulierende Clomipramin gekoppelt ist, während ein erfolgloser SE eher das Ansprechen auf das vorwiegend noradrenerge Neurone modulierende Maprotilin prädiziert (Fähndrich 1983). Da die Reaktion auf SE offenbar etwas mit der sog. Serotoninmangeldepression (Åsberg et al. 1973; Lingjaerde et al. 1982; v. Praag u. Korf 1971) zu tun hat und “imipramine binding” über die Regulierung der Aufnahmekapazität von Serotonin in die Präsynapse ebenfalls mit diesem Depressionstyp zusammenhängen dürfte, liegt es nahe zu untersuchen, ob zwischen dem biochemischen Merkmal “imipramine binding” und dem Ansprechen auf SE Zusammenhänge bestehen. Wenn tatsächlich serotonerge Mechanismen beiden Vorgängen gemeinsam sind, müßten folgende Hypothesen gelten:

Es gibt depressive Patienten mit einem Serotonindefizit im ZNS (Serotoninmangeldepression). Bei ihnen ist die Zahl der Imipraminbindungsstellen erniedrigt. Sie sprechen besonders gut auf Schlafentzug an.

2)
Es gibt depressive Patienten ohne einen Serotoninmangel im ZNS, die evtl. in anderen Transmittersystemen, z.B. dem noradrenergen System, gestört sind; deren Imipraminbindungskapazität liegt im Normbereich. Sie sprechen auf SE nicht oder nur gering an oder merken eine Besserung erst am 2. Tag nach SE (Fähndrich 1983; Wirz-Justice et al. 1979).
