Einfluß des Schweregrades auf die diagnostische Assoziation von Dexamethason-Nonsuppression und endogener Depression

Zusammenfassung

In einigen Untersuchungen des Dexamethasonsuppressionstests (DST) bei depressiven Patienten hat sich bei Dexamethasonnonsuppressoren ein signifikant höherer Schweregrad der Depressivität gezeigt als bei Dexamethasonsuppressoren (Aggernaes et al. 1983; Kasper u. Beckmann 1983; Klein et al. 1984; Rabkin et al. 1983; Reus 1982). Dieser Befund konnte in anderen Untersuchungen nicht bestätigt werden (Arnes et al. 1984; Amsterdam et al. 1982; Brown u. Shuey 1980; Carroll et al. 1980; Mendlewicz et al. 1982; Nasr u. Gibbons 1983; Rudorfer et al. 1982; Saleem 1984; Yerevanian et al. 1984). Es fragt sich, in welcher Beziehung diese widersprüchlichen Befunde zu jenen Befundunterschieden stehen, die über die Assoziation einer Dexamethasonnonsuppression mit dem Vorliegen einer endogenen Depression erhoben wurden. Es ist denkbar, daß Befunde eines häufigeren Auftretens einer Dexamethasonnonsuppression bei endogenen Depressionen lediglich Ausdruck eines im Vergleich zu nichtendogenen Depressionen höheren Schweregrades der Depression sind. Wir sind dieser Frage im Rahmen einer polydiagnostischen Untersuchung des Dexamethasonsuppressionstests nachgegangen (s. auch Philipp et al., S. 90 in diesem Band; Philipp et al., im Druck).
